Claim Procedure

 a.) If seeking treatment within our Direct Billing Hospitals Network, please bring:
–  Insurance Card
–  Photo ID / Passport

Note: 

In case you need assistant on medical card, please contact Victoria Insurance Broker to send you a photo of your card by call or WhatsApp us at: 865 30 5142 552; 85620 56097878; 85620 56107979 or email:  info@vicinsurancebrokers.com.

b.) If seeking treatment out of our Direct Billing Hospitals Network, please bring:

– Non-Direct Billing Claim form (download here) to the doctor to complete the form.

– If the doctor is not willing to complete Part B of form, it can be replaced by the photocopy of the diagnosis.

The completed of the form (Part A & B) or just Part A together with photocopy of the diagnosis can be directly submitted to:
